Hey guys!

I'm wanting to submit an art portfolio to become an Art Studio major and there's a few things I need help with! I've never done this before and I want to make this perfect. So if you have any knowledge whatsoever about doing one of these things, I'll appreciate it so much!

1.) One of the requirements for this submission is a "Statement of Application" where I have to explain my art background and the reasons for wanting to major in it. Is this like a cover letter? If so, how should I set this up? I know some people are super picky about the letter being in the "right" format. So I'm not sure who I should address this to like, "Dear whoever this may concern, blah blah blah..." or "Dear Faculty Admission Panel" (that's the only name it gave me for the people who are going to judge the portfolio.)

2.) I also need to include an Art Slide Identification Sheet that includes titles, materials, date, dimensions and any other relevant information. What is this suppose to look like? Table of contents? Idk help! XD
